Scheduling Information

Our patient hours are…

• Monday 8:00a.m. – 5:00p.m.
• Tuesday 12:00p.m. – 8:00p.m.
• Wednesday 8:00a.m. – 12:00p.m.
• Thursday 8:00a.m. – 5:00p.m.

We will schedule your appointment as promptly as possible. If you have pain or an emergency situation every attempt will be made to see you quickly.

At the time of your initial phone call, we will gather some basic information and schedule your first visit. Dental and medical history forms will be mailed to you prior to your appointment.

By completing these forms at home you can easily record any medications you are taking accurately. Please bring these completed forms to your initial visit. (***please note that you can print out your patient forms on this web page, click patient registration and mail or fax it to us). We will also contact your general dentist to obtain any x-rays that may help in your examination.

To control the growing cost of care, we ask our patients to respect our need for at least 48 hours notice for changes in appointments.

All patients under the age of 18 must be accompanied by an adult.

Our office can be reached for emergencies, 24 hours through our answering service.
